For Veterinarians

Thank you for your interest in our services. All patients evaluated at Colorado Pet Rehabilitation require a veterinarian’s referral. In collaboration with you, our staff will design a tailored rehabilitation plan that will restore the patient to optimal function. Please complete the Veterinarian Referral Form below and mail or fax it to us, along with any pertinent medical records and radiographs (contact us). You may also send this information with the client for his or her initial visit.
